Document Questionnaires

1️⃣ Why Use Questionnaires?

Whenever you need to perform a review with a consistent set of questions—like supplier due diligence, employee onboarding, or compliance checksdocument questionnaires provide a structured solution.


📌 Questionnaires ensure repeatability, auditability, and easy integration with document types.



2️⃣ Creating a Questionnaire

Questionnaires are created inside Workspace settings and later assigned to document types.


① Add a New Questionnaire

Start from the Questionnaires section in Workspace settings.


② Questionnaire Title

Enter a title you’ll later use to link this questionnaire to a document type.

③ Categories

Define categories to group related questions for clarity.

④ Questions

Map your questions to categories, define wording, and choose the reply type.


Each question can include:

  1. Category – Group from the previous step.
  2. Question – The wording.
  3. Answer type example: Custom text with free text replies and optional default.
  4. Default reply – Pre-filled text for guidance.
  5. Answer type example: Content of existing document(s),
  6. Link to the document in Brainframe.
  7. Answer type example: Yes/No – Simple binary responses.


📌 Once complete, questionnaires can be linked to document types (see Document Types & Templates).



3️⃣ Using Questionnaires

When assigned to a document type, all documents of that type gain a Q&A tab.



Features include:

  1. Questions grouped by category.
  2. Each question shows its status.
  3. An action button lets you answer based on the reply type.


📌 The rest of the document behaves as normal—questionnaires are an added layer.



4️⃣ Managing Answers


Options for administrators:

  1. View replies – Review all provided answers.
  2. Add to workbench – If more information is needed, or the questionnaire requires more work, you can directly add it to your tasks from here.
  3. Locked answers – Further edits to responses are prohibited.
  4. Lock answers – Forbid further modifications.
  5. Print results – Generate a consolidated Q&A document.


💡 Locking answers ensures questionnaires are frozen in time for audit purposes.



5️⃣ Best Practices

  • 📌 Always group questions into categories for readability.
  • 💡 Use default replies for common answers to speed up input.
  • 🔍 Link questionnaires to document types that require recurring validation (e.g., supplier risk).
  • 🖨️ Use the print results option to archive responses as formal evidence.
  • 🚨 Lock answers once reviews are complete to prevent accidental changes.
